Counseling Referrals: Knowing who to call and making phone calls to community providers can be challenging. Your Case Manager will assist you in finding appropriate referrals to providers in the community for both individual and group therapy. 

Community Resource Referrals – Your Case Manager will help you navigate and understand available community resources. Resources include food, clothing, housing, partner social service agencies, etc. Your Case Manager will help ensure that all of your needs are met and that you are properly connected.
